2. Fill in the relevant information like
   1. Vehicle Number
   2. Make
   3. Chassis No. (Last 5 digits)
   4. Engine No. (Last 5 digits)
   5. Check service as per requirement in form, Check point 5 for Issuance of new Colour Coded Sticker
3. Photocopy of the RC (1 Front & Back Side) Self Attested
4. Photocopy of the Valid Insurance Self Attested.

Fee: 27 Rs. They may ask for the 200 Rs pending dues for some series of vehicle.
